Introdução: O tumor de Brenner é uma neoplasia ovariana rara, correspondendo a aproximadamente 1–5% dos tumores do ovário, sendo predominantemente benigno, unilateral e mais frequente em mulheres na pós-menopausa. Objetivo: Relatar um caso de tumor de Brenner gigante em paciente de 56 anos. Relato do caso: Mulher de 56 anos apresentou tumoração abdominal volumosa, de crescimento progressivo, associada a desconforto e episódios recorrentes de cólica abdominal. Os exames de imagem iniciais sugeriram mioma uterino, porém exames subsequentes indicaram origem anexial. A paciente foi submetida à laparotomia mediana com exérese completa da massa tumoral. O exame histopatológico confirmou tumor de Brenner benigno. A evolução pós-operatória foi satisfatória, sem evidência de recidiva durante o seguimento. Conclusão: Embora geralmente pequenos e assintomáticos, os tumores de Brenner podem apresentar crescimento volumoso e sintomatologia significativa. O diagnóstico definitivo baseia-se no exame histopatológico, e o tratamento cirúrgico mostrou-se eficaz no presente caso.
