BIA-ALCL E DIAGNÓSTICO DIFERENCIAL COM LINFOMA ANAPLÁSICO DE GRANDES CÉLULAS SISTÊMICO PRIMÁRIO: RELATO DE CASO

Relatamos o caso de uma paciente submetida à mamoplastia de aumento em 2016 que evoluiu após 4 anos com sintomas de adenomegalia das cadeias supraclavicular, axilar e cervical esquerda assim como flogose na mama ipsilateral. O caso relatado e publicações levantadas trazem à luz a discussão de um caso cuja principal hipótese diagnóstica aventada foi de BIA- ALCL. A paciente apresentava histórico prévio de mamoplastia de aumento com colocação de implantes microtexturizados somado aos achados clínicos, exame físico, cadeias linfonodais acometidas e exames de imagem com alterações compatíveis em topografia de mama esquerda. Os marcadores obtidos através da biópsia com imuno-histoquímica de tecido linfonodal foram CD30 positivo (forte e difuso) e ALK negativo, compatíveis com o diagnóstico do BIA- ALCL. Porém, o desfecho atípico obtido com os achados incaracterísticos da peça cirúrgica após remoção dos implantes e capsulectomia total bilateral será o enfoque do nosso relato de caso.
