SOFT TISSUES COVERAGE IN GUSTILO ANDERSON III B LEG FRACTURES WITH KEYSTONE FLAP: CASE REPORT

The study presents different clinical cases with a diagnosis of a Gustilo-Anderson III B type leg fracture, with defects in planes of moderate dimensions, and exposure of bone, osteosynthesis material or both, which were treated with a Keystone-type flap. Methodologically, it is a case report; descriptive, observational, retrospective. Three patients were studied, with an average age of 53 years. The status of the perforators was assessed with portable arterial Doppler. Type II A Keystone flap was performed in the reported cases. Complete closure of the defect that was intended to be covered, and primary closure of the donor area were obtained; short operative time without major complications. We conclude that the Keystone flap may be an appropriate option in terms of its design, with good vascular supply, short duration of surgical time, minimal donor site morbidity, little postoperative pain, rapid recovery, and good cosmetic results.
