Introduction: The desire for breast aesthetics has long been described as a relevant characteristic in improving the self-esteem of the female audience. In their quest to obtain it, they seek to plastic surgeons to perform procedures such as mastopexies and, or breast reduction. Objective: To present the surgical technique for managing the volume of the lower pole, which aims to balance the disproportion between lateral and medial volume, taking the lateral surplus to the medial absent volume. Methods: 135 cases of patients with breast ptosis were analyzed, opting for the surgical technique of inverted T mastopexy with or without the placement of a prosthesis to reconstitute the lower pole. Results: With adequate preoperative evaluation and use of precise surgical techniques, excellent results were obtained, as well as the fulfillment of patients expectations. Conclusion: The treatment of breast ptosis requires a combination of careful preoperative analysis and complex techniques, in order to obtain lasting and harmonious results.
