Abstract

O câncer colorretal é uma das neoplasias malignas mais prevalentes e letais globalmente. A ressecção endoscópica mucosa é uma técnica para remover lesões colorretais grandes e potencialmente pré-malignas. O objetivo deste estudo foi analisar os aspectos endoscópicos e histopatológicos de lesões submetidas à ressecção mucosa durante exame de colonoscopia em um serviço de referência privado em Santa Catarina. Trata-se de um estudo retrospectivo com coleta de dados secundários de pacientes submetidos a mucossecotomia entre janeiro de 2016 e julho de 2020. Dados endoscópicos e histopatológicos foram compilados e analisados. Os resultados indicam que o adenoma tubuloviloso de baixo grau apresentou tamanho médio maior que o adenoma tubular de baixo grau e pólipos hiperplásicos. Lesões de 0 a 9 mm associaram-se a adenoma tubular de baixo grau, de 10 a 19 mm a lesão serrilhada séssil, e ≥ 30 mm a adenoma tubuloviloso de baixo grau. Lesões serrilhadas sépteis localizavam-se predominantemente no cólon ascendente, enquanto adenomas tubulovilosos de baixo grau estavam principalmente no reto. Tumores de crescimento lateral (LST) granulares associaram-se a adenomas tubulares e tubulovilosos, enquanto LSTs não granulares associaram-se a lesões serrilhadas sépteis e pólipos hiperplásicos. O padrão de pits de Kudo tipo II associou-se a pólipos hiperplásicos, tipo IIO a lesões serrilhadas sépteis, tipo IIIL a adenomas tubulares, e tipos IV/IIIL+IV a adenomas tubulovilosos. Conclui-se que a ressecção mucosa se mostrou eficaz para remoção segura de lesões colorretais com histopatologia avançada, permitindo a excisão de lesões pré-malignas em estágios iniciais.
