DESPRESCRIÇÃO EM IDOSOS E DESFECHOS CLÍNICOS

UMA REVISÃO SISTEMÁTICA

Resumen

Introdução: A polifarmácia é frequente na população idosa e está associada ao aumento do risco de eventos adversos, hospitalizações, quedas e declínio funcional. Nesse contexto, a desprescrição tem sido proposta como estratégia para otimizar a farmacoterapia e reduzir danos relacionados ao uso excessivo de medicamentos. Objetivo: Avaliar os efeitos da desprescrição de medicamentos sobre desfechos clínicos em idosos quando comparada ao cuidado usual. Métodos: Realizou-se uma revisão sistemática da literatura conforme as recomendações PRISMA. A busca foi conduzida na base PubMed utilizando descritores relacionados à desprescrição, idosos, polifarmácia e desfechos clínicos. Foram incluídos estudos publicados nos últimos 10 anos envolvendo indivíduos com 60 anos ou mais submetidos a intervenções de desprescrição. Após aplicação dos critérios de elegibilidade, 12 estudos compuseram a amostra final. Resultados: Os estudos demonstraram que a desprescrição foi eficaz na redução da carga medicamentosa e, em alguns casos, associou-se à melhora da qualidade de vida e da funcionalidade. Entretanto, os resultados referentes à mortalidade, hospitalização, quedas e eventos adversos mostraram-se heterogêneos, sem evidências consistentes de benefício ou prejuízo clínico significativo. Conclusão: A desprescrição representa uma estratégia promissora para o manejo da polifarmácia em idosos, contribuindo para a individualização do tratamento e redução da carga medicamentosa. Contudo, seus impactos sobre desfechos clínicos relevantes permanecem variáveis, indicando a necessidade de novas pesquisas.
