PREVALÊNCIA DE MULTICOMORBIDADES CARDIOMETABÓLICAS E ANÁLISE DE SOBREVIDA EM PACIENTES DIAGNOSTICADOS COM COVID-19

O objetivo foi identificar prevalência de multicomorbidades em pacientes diagnosticados com COVID-19 (obesidade, diabetes melitos e doenças cardiovasculares) e relacionar a presença destas comorbidades com a necessidade de internação e óbito. Por último, verificar a influência das comorbidades na sobrevida dos pacientes com COVID-19. Trata-se de um estudo de coorte retrospectiva de 15042 casos confirmados. A prevalência de comorbidades associadas foi de 14,5%. Os pacientes com maior número de comorbidades tiveram maior proporção de internações (p<0,001) e óbitos (p<0,001). Quando houve a presença concomitantemente de doença cardiovascular (DCV) com diabetes mellitus (DM) (OR= 5,52, p<0,001), obesidade e diabetes (OR= 4,22, p<0,001) e diabetes isoladamente (OR=3,09, p<0,001) houve mais chance de internação. No caso dos óbitos, a presença de obesidade e diabetes (OR=7,75, p<0,001), obesidade e doença cardiovascular (OR=7,23, p<0,001) e obesidade isoladamente (OR=3,32, p<0,001) aumentaram a chance de óbito. Os pacientes com DCV (média de 48 dias, IC95%: 43-49), diabetes (média de 95 dias, IC95%: 76-114), obesidade e DM associadas (18 dias, IC95%: 17-20), DCV e DM (99 dias, IC95%: 78-121), obesidade e DCV (média 21 dias, IC95%: 20-22) e Obesidade, DCV e DM associadas (23 dias, IC95%: 21-25) tiveram a sobrevida reduzida. A regressão de Cox mostrou que os pacientes com obesidade e DM (HR= 33,53, IC95%: 13,35-84,24) foram os tiveram maior risco de mortalidade. Em conclusão, as comorbidades associadas influenciaram na hospitalização e óbito dos pacientes. A sobrevida dos pacientes com DM, DCV e obesidade foi menor e o risco de óbito foi maior exceto, para obesidade que permaneceu semelhante aos pacientes sem comorbidades.
