DEPRESSÃO EM PACIENTES COM DIABETES MELLITUS TIPO 2: UMA REVISÃO NARRATIVA

A depressão em pessoas com diabetes mellitus tipo 2 (DM2) é muito prevalente e está relacionada a uma maior dificuldade no controle e enfrentamento do diabetes, o que torna fundamental a triagem e o manejo adequado dessa condição. O presente estudo tem por objetivo realizar uma revisão narrativa dos estudos publicados sobre a epidemiologia, fisiopatologia, impacto clínico, metabólico e psicossocial, triagem e tratamento da depressão em pessoas com DM2. Foram observadas altas taxas de comorbidade entre depressão e DM2, com uma relação bidirecional entre as duas condições, explicada por mecanismos biológicos e psicossociais compartilhados. Os fatores associados à maior prevalência de depressão no diabetes foram sexo feminino, menor escolaridade, diagnóstico recente do diabetes, baixa atividade física e uso de insulina. Quanto aos impactos da depressão no DM2, observou-se dificuldade de adesão ao tratamento, pior controle glicêmico, maior risco de complicações e mortalidade, e redução da qualidade de vida. No que tange ao rastreio, a triagem rotineira tem sido recomendada, utilizando ferramentas validadas. Entre os tratamentos, diversas modalidades de intervenção, incluindo abordagens psicossociais e farmacológicas, estão associadas a uma melhora importante dos sintomas depressivos e do controle do diabetes.
