RECONSTRUÇAO FUNCIONAL DO POLEGAR APOS QUEIMADURA ELETRICA EM CRIANÇA: RELATO DE CASO

As lesões elétricas na infância, embora incomuns, podem gerar sequelas devastadoras, especialmente quando envolvem o polegar — estrutura responsável por cerca de 40% da funcionalidade da mão. Relatamos o caso de um paciente masculino, 2 anos, vítima de queimadura elétrica com lesão de entrada no polegar esquerdo e saída no abdome. Inicialmente considerado para amputação, foi transferido a centro terciário, onde apresentou queimadura de terceiro grau no polegar e segundo grau em abdome e coxa. A abordagem incluiu desarticulação da falange distal, cobertura provisória com tecido palmar e, posteriormente, enxertia de pele total com área doadora na coxa. A ferida cicatrizou sem complicações e o movimento de pinça foi preservado. A reconstrução do tendão flexor foi planejada. Este caso destaca a importância da abordagem reconstrutiva escalonada e do planejamento multidisciplinar para preservação funcional em queimaduras graves na infância.
